Whether your knees ache after running or stiffness has gradually appeared with age, experts explain the supplements most likely to make a difference.

Recently, joint supplements have become one of the biggest categories in wellbeing, but they're also one of the easiest places to spend money without seeing much in return. Why? They certainly work, but they may be acting on the wrong issue. Aching knees after years of running, stiffness caused by osteoarthritis, and pain linked to inflammatory conditions can all feel remarkably similar, while responding very differently to supplementation.
Current research reflects that complexity. Some ingredients, including certain forms of collagen and curcumin, have shown modest benefits for people with osteoarthritis. Others remain far more divisive, while some of the most expensive formulas contain long ingredient lists with little evidence that combining them delivers better results.
As sports nutritionist Rob Hobson, MSc, AfN, SENr, explains: "most of the evidence relates to osteoarthritis, particularly knee osteoarthritis. We can't automatically apply these findings to every type of joint pain." Meanwhile, Dr Amos Ogunkoya, MBBS, MRCGP, MSc, also cautions against treating all joint pain as the same condition. "It isn't correct to assume all pain is 'wear and tear'," he says.
Looking for a single "best" joint supplement misses the point. Instead, it helps to understand what's most likely causing the pain, and then finding the best course of action. So, that's exactly where we'll start.
Ask either of our experts which supplement they recommend first, and both will start with the diagnosis, not collagen, glucosamine or turmeric. "The most important part of an assessment is usually the history and then examination," says Dr. Ogunkoya. "It isn't correct to assume all pain is 'wear and tear'."
That's because joint pain describes a symptom rather than a single condition. Osteoarthritis, tendon injuries, inflammatory arthritis, and even pain referred from elsewhere in the body can all produce similar aches while responding very differently to treatment.
Getting a clearer idea of what's driving your symptoms does two things. It helps set realistic expectations about what supplements can achieve, and it makes it much easier to decide whether they're worth trying in the first place. Here's how Dr. Ogunkoya approaches the most common causes of joint pain.
If pain has gradually developed over months or years, feels worse during weight-bearing activity, and improves with rest, osteoarthritis becomes more likely. Morning stiffness is common but usually settles within around 30 minutes. This is also the group studied most extensively in supplement research.
"The truth is supplements have their greatest effect in those with mild symptoms and their effects are usually modest," says Dr. Ogunkoya. "The majority of the evidence supports treatment of the knee." That reflects Hobson's interpretation of the evidence too. Most clinical trials investigating collagen, curcumin and glucosamine have focused on people with knee osteoarthritis, making it difficult to assume the same benefits extend to every aching joint.
Pain that's highly localised, linked to a particular movement, and consistently appears when loading the same area is more suggestive of a tendon injury than cartilage wear. Dr. Ogunkoya explains that tendon pain is often reproducible with the same movement or pressure stimulus and usually benefits more from appropriate rehabilitation, progressive strength training, and managing training load than from supplements alone.
Joint pain that comes with prolonged morning stiffness, swelling, warmth, or several painful joints deserves a different approach altogether.
According to Dr. Ogunkoya, supplements are much less likely to help inflammatory arthritis, gout, fractures, or referred pain from the spine because these conditions require accurate diagnosis and targeted treatment rather than self-management with supplements.
He also advises seeking medical assessment if joint pain lasts longer than six to 12 weeks without improving, or sooner if it's accompanied by weight loss, night sweats, an inability to bear weight, or signs of infection. Blood tests such as CRP, ESR, a full blood count, vitamin D, iron studies, and thyroid function may also help identify underlying causes where appropriate.
Choosing the best joint supplement starts with having realistic expectations. "I'd be really cautious about products that promise to rebuild cartilage or reverse arthritis because that is not what the evidence shows," says Hobson.
Dr. Ogunkoya hears the same claim in clinic. "The biggest misconception I come across is that supplements can rebuild worn-out cartilage or reverse arthritis. The evidence simply doesn't support this," he says.
That doesn't mean supplements aren’t a good idea. Instead, both experts describe them as a way to help manage symptoms in the right people, with the strongest evidence seen in those with mild osteoarthritis, particularly affecting the knee, where some ingredients have been shown to improve pain, stiffness, and physical function. People shouldn't expect pain to disappear. Instead, the research suggests some people experience modest but meaningful improvements in pain, stiffness and day-to-day function.
Hobson consistently places two ingredients ahead of the rest: hydrolysed collagen peptides and curcumin. Both have shown modest improvements in pain and physical function in people with osteoarthritis, although neither rebuilds cartilage or reverses the condition. Here's what to know before choosing either.
Collagen has become one of the most popular supplements for joint pain because it's the main structural protein found throughout cartilage, tendons, and ligaments. As natural collagen production declines with age, researchers have investigated whether replacing some of those collagen peptides can improve joint comfort and mobility.
According to Hobson, hydrolysed collagen peptides currently have the strongest evidence, with studies showing modest improvements in joint pain and physical function in people with osteoarthritis. He also notes that current research doesn't show marine collagen is more effective than bovine or chicken collagen, making the formulation more important than the source itself.
Choose: Hydrolysed collagen peptides.
Take: 5-10g daily.
Trial: At least three months before deciding whether it's helping.
Remember: Collagen may improve pain and physical function, but it hasn't been shown to rebuild cartilage.
Curcumin attracts attention because of its anti-inflammatory properties, but buying the right formulation is far more important than buying the highest dose. Unlike turmeric powder, however, curcumin is naturally difficult for the body to absorb. Hobson ranks curcumin alongside collagen as one of the strongest-supported supplements for osteoarthritis. Most of the positive studies have investigated standardised curcumin extracts with enhanced bioavailability rather than turmeric powder itself.
"The biggest consideration isn't turmeric itself, it's the formulation," he explains. "Turmeric powder on its own is poorly absorbed."
Choose: A standardised curcumin extract with enhanced bioavailability as opposed to a turmeric powder.
Take: The serving size recommended by the manufacturer, provided it's consistent with clinically studied doses.
Trial: Around 4-8 weeks.
Remember: Most successful studies have used enhanced curcumin extracts. Turmeric powder isn't equivalent because it's naturally poorly absorbed.
Glucosamine has been recommended for osteoarthritis for decades, but it's also one of the most debated joint supplements. "I wouldn't dismiss glucosamine altogether, but I also wouldn't use it as my first choice based on the current evidence," says Hobson.
One reason the research has been difficult to interpret is that studies haven't all investigated the same form. Hobson explains that prescription-grade crystalline glucosamine sulphate has generally produced better results than many over-the-counter formulations. That distinction is reflected in NICE guidance, which no longer routinely recommends glucosamine for osteoarthritis.
A lot of products sold as "glucosamine" actually contain glucosamine hydrochloride, meaning they may not offer the same evidence as crystalline glucosamine sulphate.
Choose: Glucosamine sulphate rather than glucosamine hydrochloride.
Take: The serving size recommended by the manufacturer, provided it's consistent with clinically studied doses.
Trial: Around 2-3 months before deciding whether it's helping.
Remember: If you're starting from scratch, Hobson would generally choose hydrolysed collagen peptides or a well-formulated curcumin extract before glucosamine.
Some ingredients show promise but aren't supported by the same volume or consistency of evidence. According to Hobson, UC-II (undenatured type II collagen) is one of the more promising emerging ingredients, although there are far fewer high-quality studies than for hydrolysed collagen peptides or curcumin. He also notes that omega-3 fatty acids have shown stronger evidence in inflammatory arthritis than osteoarthritis, while oral hyaluronic acid has a much weaker evidence base than many people expect.
Dr. Ogunkoya adds an important clinical distinction. Inflammatory arthritis requires an accurate diagnosis and disease-specific treatment, meaning supplements should be viewed as complementary rather than a replacement for appropriate medical care.
Combination supplements promise to cover every angle, often blending collagen, glucosamine, MSM, chondroitin, turmeric, and hyaluronic acid into a single formula. Hobson isn't convinced that's automatically an advantage.
"There isn't strong enough evidence that combination supplements consistently outperform single, evidence-based ingredients," he says. Instead, he recommends introducing one supplement at a time and giving it enough time to work before adding anything else. That makes it much easier to judge whether it's having an effect and avoids spending money on ingredients that may not be contributing anything meaningful.
Joint supplements have been studied most extensively in people with mild osteoarthritis. Outside of that, it's much harder to draw clear conclusions from the available research.
For those with osteoarthritis, supplements may have a place alongside exercise, weight management, and other lifestyle measures. If your symptoms don't fit that picture, getting the right diagnosis is likely to have a much bigger impact than changing your supplement routine.
Both experts are clear that supplements should sit alongside, not instead of, the fundamentals of joint health. For Hobson, that means eating enough protein, maintaining a healthy weight, strength training regularly, prioritising sleep and meeting your calcium and vitamin D requirements.
Dr Ogunkoya shares the same view. Depending on your symptoms, he may also investigate factors such as vitamin D deficiency, iron status, vitamin B12, folate or thyroid function, while encouraging appropriate exercise and rehabilitation alongside any supplement strategy.
Taken together, their advice is consistent. Choose supplements with evidence behind them, match them to the right type of joint pain and give them realistic expectations. No supplement can rebuild cartilage or replace the foundations that keep joints healthy over the long term.
If you have mild osteoarthritis, the current evidence points towards hydrolysed collagen peptides and well-formulated curcumin extracts. If you're considering glucosamine, choose crystalline glucosamine sulphate, as other forms haven't shown the same level of rigorous evidence.
The biggest difference isn't usually the ingredient itself, but the formulation. Choose products that match those used in clinical studies, take them consistently and give them enough time to judge whether they're making a difference.
Go for the formula that's been studied, not the one with the longest ingredient list.
